Lake Superior benefit concert dramatic as severe storms Around 5:30 this morning (July 8, 2007) a series of severe thunderstorms rolled across Lake Superior and the Upper Peninsula of Michigan with vivid lightening and later hail. This is natural sound from the storm, followed by slow motion. Then some even slower motion and photos of lightening with natural sound of the thunder. Finally a brief hail storm dropped pea-sized ice about 7:15 a.m. It was the third hail storm in Marquette County since a six-day blizzard during the first week of April 2007 dumped 5 feet of snow across northern Michigan. Protecting Lake Superior is the goal of a free benefit concert on Sunday July 15 that promises to be as dramatic as today's storm. Midwest classical musicians have just formed the Boreal Chamber Symphony to protect Lake Superior. The Boreal orchestra will makes its debut in Marquette on July 15 (international Lake Superior Day) using sand, rocks, driftwood, water from the big lake to make music. Donations go to Lake Superior Defense Fund and will be used to continue ongoing projects that protect the largest and deepest of the Great lakes.. The concert is sponsored by the Superior Watershed Partnership and the Cedar Tree Institute, Marquette non-profits that founded the interfaith Earth Keeper Initiative in 2004. The Earth Keepers have recycled or properly disposed of over 370 tons of household poisons on past three Earth Day weekends. Severe Tissue Injury with IV Promethazine The Institute for Safe Medication Practices recently warned that administering promethazine parenterally may cause more severe tissue injuries than previously recognized. Promethazine, which is commonly sold under the trade name Phenergan, has many uses including as an antihistamine, a sedative and an antiemetic. ISMP points that out promethazine is a known vesicant which is highly caustic to the intima of blood vessels and surrounding tissue. Deep intramuscular injection into a large muscle is the preferred parenteral route of administration, and is least likely to cause damage. But promethazine is also given by slow IV push in many hospitals. The IV route, along with inadvertent intra-arterial or subcutaneous administration is most likely to result in significant complications, ranging from burning and pain to paralysis, tissue necrosis, and gangrene. Sometimes surgical intervention such as fasciotomy, skin graft or even amputation is needed. ISMP describes several recent cases where severe tissue damage occurred following IV administration of promethazine. For example, in one case a 19-year-old woman who went to the emergency department with flu-like symptoms was given Phenergan IV. During the injection, she cried out in pain, and after the injection she told the nurse that her arm was still in significant pain and that she felt "something was wrong." The nurse reassured her and then left the room. The patient's arm and fingers became purple and blotchy and then over the next several weeks eventually turned black and shriveled. Her thumb, index finger, and top of her middle finger eventually had to be amputated. ISMP points out that the product labeling makes several recommendations to reduce the risk of these adverse events. They include giving the drug in concentrations no greater than 25 mg/mL, administering the drug at a rate no greater than 25 mg/minute, injecting the drug through the tubing of an infusion set that's running and known to be working satisfactorily, and stopping the injection immediately if the patient reports burning in order to investigate whether there might be intra-arterial placement or perivascular extravasation. In addition to the manufacturer's recommendations, ISMP lists about a dozen other strategies to prevent or minimize tissue damage when giving IV promethazine. Here are several suggestions. • Stock promethazine only in the 25 mg/mL concentration (not the 50 mg/ml) since this is the highest concentration of promethazine that can be given IV. • Consider 6.25 to 12.5 mg of promethazine as the starting IV dose, especially for elderly patients. • Give the medication only through a large-bore vein and check the patency of the access site before administering. • Administer IV promethazine through a running IV line at the port furthest from the patient's vein. • Consider administering IV promethazine over 10-15 minutes. • Before administering the drug, tell patients to let you know immediately if burning or pain occurs during or after the injection. Severe Thunderstorm June 8, 2008 On Sunday, June 8, 2008, the National Weather Service issued a Severe Thunderstorm Warning for Southwestern Suffolk County in Southeast New York State. At 6 PM, the thunderstorm passed over my house (located in the warned area) with wind gusts around 60 mph, hail slightly smaller than pea-sized, a torrential downpour, lightning, thunder, and even a rainbow. The storm left its mark. Lightning struck through the roof of a home in Lindenhurst, and numerous trees were blown down. Lawn chairs were blown into the canal behind my house. The roof of a bath house at a local beach was completely ripped off by the winds as well. Numerous power outages were seen across Long Island. There was an unconfirmed tornado report in East Meadow. Compared to all the storms I have seen here in Lindenhurst in the past 10 years or so, this would have to be on the list of the top 10 most severe storms. Case worker wanted Baby P in care (BBC NEWS) A senior social worker and police did not want Baby P returned home where he suffered severe abuse, the BBC's Panorama has learned. It found social worker Sylvia Henry wanted him to go into foster care after he was taken to a north London hospital in 2006 with non-accidental injuries. But her bosses decided Baby P should be looked after by a family friend. Haringey Council has apologised for not doing more, but a statement disputes some of the claims made by Panorama. Baby P died in August last year. His mother and two men are due to be sentenced next month for causing or allowing his death. In response to the case, Children's Secretary Ed Balls has told the House of Commons he will set out legislation on Tuesday to strengthen local arrangements to promote the safety and well-being of children across Britain. 'Rough play' The matter of Baby P's care arose after doctors at Whittington Hospital found he had non-accidental injuries in December 2006. He was subsequently placed in the care of Angela Godfrey, a friend of his mother. But Ms Henry, the senior social worker at the time, had wanted Baby P to be taken into care and had found him a foster placement. She said she had been "very reluctant" to let Baby P be looked after by Ms Godfrey, but she was bound by the 1989 Children Act to explore placement options with family and friends. In a witness statement given to the police and seen by the programme, Ms Henry said: "My impression of Angela was that she believed the local authority were over-reacting and that the explanation for Baby P's injuries were those of his mother's, that they were caused by rough play and by his head-banging." She also revealed Ms Godfrey asked for "a large sum of money, possibly £320 per week" to look after the child, and continually pressed for his return home. Baby P suffered no injuries while in Ms Godfrey's care. Police began investigating Baby P's case after he was put on the child protection register. But with no new evidence to support a prosecution, services manager Clive Preece instructed that the boy should go back home. 'Frank exchange' However, Ms Henry said she had delayed this because police had expressed a similar opinion to her own, that the child should remain "out of the care of his mother". In a statement, Haringey Social Services denies that Mr Preece overruled social workers. "No concerns were raised regarding placement with Angela Godfrey at the time of the placement," it said. The programme has also learned that police and social services had a serious disagreement over the care of Baby P after he was admitted to North Middlesex Hospital in June 2007 with more non-accidental injuries.
