American Crappy Plane Crashes (5) American Airlines Flight 96 was a regular McDonnell Douglas DC-10-10 commuter flight operated by American Airlines, with a scheduled route from Detroit, Michigan to Buffalo, New York. The flight suffered explosive decompression, due to cargo door failure, on 12 June 1972 while flying over Windsor, Ontario; it is thus sometimes referred to as the Windsor incident.[1] The failure of the cargo door caused the partial collapse of the passenger compartment floor. The sagging floor ruptured some hydraulic lines, pulled the rudder control cable to its maximum left position, and severed controls to engine number two leading to serious problems operating the aircraft.[2] The plane had no rudder power, and little responsiveness in the elevators or ailerons. However the crew, led by Captain Bryce McCormick, were able to apply differential thrust in the DC-10's wing engines to turn the aircraft, and use what elevator control they had to maintain vertical stabilization. It happened that while converting to the DC-10, McCormick had practiced in a simulator controlling the plane in this fashion, in the worst-case scenario of a hydraulic failure.[1] A similar technique was used in 1989 following a complete loss of hydraulic pressure on another DC-10, United Airlines Flight 232. Despite the aircraft's being severely damaged, the crew made a successful emergency landing at Detroit, and all people on board evacuated safely. The incident highlighted a serious design flaw in the cargo doors of DC-10 aircraft whereby the door could appear closed but be ineffectively latched.[3] The fault was not satisfactorily repaired, and a subsequent cargo door blow-out occurred in another DC-10 two years later, on Turkish Airlines Flight 981, killing all 346 on board. At the time, the Turkish Airlines accident was the deadliest aviation disaster in history. Many commentators subsequently blamed the aircraft manufacturer, McDonnell Douglas, and other aviation authorities, for failing to learn lessons from the Flight 96 incident. Although there had been some redesign of the DC-10 cargo door system, it had only been implemented voluntarily and haphazardly by various airlines. If the warning signs of Flight 96 had been heeded, the Turkish Airlines disaster might have been avoided. Istanbul that morning, landing at Paris's Orly International Airport just after 11:00 AM local time. The aircraft, a McDonnell Douglas DC-10, was carrying just 167 passengers and 13 crew members in its first leg. 50 passengers disembarked to Paris. The flight's second leg, from Paris to London's Heathrow Airport, was normally underbooked, but owing to a strike by British Airways employees, many London-bound travelers who had been stranded at Orly were booked onto Flight 981. There were 17 English rugby players who had attended a France-England match the previous day; the flight also carried four British fashion models, 48 Japanese bank management trainees on their way to England, as well as passengers from a dozen other countries. The aircraft departed Orly at around 12:30 PM for its flight to Heathrow. It took off in an easterly direction, then turned to the north to avoid flying directly over Paris. Just after Flight 981 passed over the town of Meaux, controllers picked up a distorted transmission from Flight 981; the aircraft's pressurization and overspeed warnings were heard over the pilots' words in Turkish, including the co-pilot saying "the fuselage has burst." The flight disappeared from radar shortly thereafter, and its wreckage was later found at the Grove of Dammartin in the Ermenonville forest, close to the town of Senlis. Well, the gameplay is the demo sequence. Some games have good ones but then you have those games that just have horrible ones that make the game look worse than it is. If you are wondering why I included Mario, it's because compared to all the 100+ games I've watched it actually is kinda lame. It doesn't really show much to the game. A lot of games actually do have good demos even the crappy games so this video was sorta tough to make. I had a total of like 16 demos but I shrunk it down to 10 because some were boring to watch which will make you bored watching this video. American Crappy Technology: The Boeing 737 (1) After 15.000 people lives killed... 737 Rudder Design Changes Mandated On Oct. 7, the U. S. Federal Aviation Administration (FAA) issued an airworthiness directive (AD) mandating changes to enhance the 737 rudder control system -- expected changes that Boeing announced it was making on September 14, 2000. The modifications amplify three areas: flight crew procedures, maintenance procedures and control system design. These three steps make an airplane with an impeccable safety record even safer. The design changes the FAA mandated today have been made in agreement with a National Transportation Safety Board's (NTSB's) finding that the current rudder system could be made more "reliably redundant." Boeing agreed with that recommendation, and Boeing engineers designed new hardware that provides two separate and independent control inputs to the main power control unit. Within two months of its Sept. 14, 2000 announcement, Boeing issued cockpit procedures to its airline customers that simplify and clarify how to address a condition involving a jammed or restricted rudder. In less than a year, Boeing employed new rudder system maintenance procedures for Initial (737-100/ -200) and Classic 737s (737-300/ -400/ -500) that match procedures for the Next-Generation models (737-600/ -700/ -800/ -900). The redesign has been completed and the shipment of kits to operators is proceeding as planned. The first airplane with the design change installed in-production was delivered January 21, 2003 -- six months earlier than first projected. Boeing is providing wiring provisions ahead of hardware (main rudder power control unit and supporting parts) availability to give airlines more flexibility to incorporate the redesign, and as kits are ordered, Boeing is fulfilling these orders per the operator's schedule. The first wire provisioning kits were shipped to operators of Initial and Classic 737s in February 2002. Operators of Next-Generation 737s began receiving wire provisioning kits in August 2002. Boeing began shipping hardware kits to Next-Generation 737 operators in June 2003 and to Classic 737 operators in July 2003. Operators of Initial 737 models began receiving their kits in September 2003. As of March 2006, 1,051 hardware kits have been shipped to Initial and Classic 737 operators and 696 hardware kits have been shipped to Next-Generation 737 operators. "The 737 is a remarkably safe and reliable airplane," said Carolyn Corvi, vice president and general manager of the 737 program. "It has been the workhorse of the world's jet fleet. Given this record, we are assured that the rudder modifications add to its robustness, and make a safe system even safer." At the end of February 2006, Boeing had orders for 6,160 737s and had delivered 5,009 737s to more than 200 operators in more than 100 countries around the world.
